We are presenting a case ofE.R., a 45-year-old female, who was admitted because of dyspnea. Two months prior to admission, she had cough and fever, which resolved after intake of antibiotics. These recurred after one month and she was admitted at the Makati City Hospital because of difficulty of breathing. She was managed as a case of Infective Endocarditis and RCSOV and CHF. However, she deteriorated and was transferred to our institution. Upon admission, the patient was congested with note of continuous murmur over the left parasternal area. She was diagnosed to have rupture coronary sinus of valsalva and infective endocarditis. She was referred to surgery but aggressive medical management was opted. In spite of these and the renal replacement therapy, patient did not improve and eventually expired on the eleventh hospital day
