Acute effects of a typical regional Filipino diet on endothelial function in healthy subjects.

INTRODUCTION: Endothelial dysfunction is a basic abnormality in coronary artery disease, hypertension and diabetes. This prospective study was designed to find out the influence of different regional Filipino diets on endothelial function.

METHODOLOGY: Twenty six healthy subjects were recruited. They were all <45 years, physically active, non-hypertensive, non-diabetic, non-smoker and with normal body mass indices. Baseline endothelial function was taken twice at the vascular laboratory of the St. Luke's Medical Center Heart Institute by measuring the brachial vasoreactivity. Five different regional diets were prepared by the Department of Nutrition of the St. Luke's Medical Center. Each of the diet contained specific of carbohydrates, protein, fat, cholesterol, sodium, and calories. Diets were given by rotation at one week interval until each subject has completed the five diets. The brachial artery vasoactivity was remeasured 4 hours after eating. The resultant post-prandial measurements were compared with the baseline level for each subject.

RESULTS: A total of nineteen subjects were able to finish the whole study. Wilcoxson-Signed Ranks Test a p-value of .05, four of the five regional diets produced significant reduction in the vasodilation of the brachial artery. The Tagalog, Kapampangan, Bicolano and Ilocano recipe had significant reduction in the vasodilation of the brachial artery which reflects endothelial function. Among the four diets which produced significant results, the Kapampangan diet showed the least vasodilation as compared to baseline.

CONCLUSION: Among the five different regional diets included, the one with the highest cholesterol content (which is the Kapampangan diet) had the -least vasodilation of the brachial artery. A single high cholesterol transiently impairs endothelial function.

CLINICAL SIGNIFICANCE: This study supports the value of both a low-fat and low-cholesterol diet as an important component for preventing and treating coronary artery disease, even in persons with normal fasting cholesterol levels.
