These syndromes including frailty, sarcopenia, cognitive impairment, and anorexia of aging either in isolation or combination are associated with increased risk of adverse outcomes including hospitalization, falls, functional decline, institutionalization, and mortality 26–29. Current healthcare is based on the traditional disease-based model which is no longer sustainable in countries with a fast-aging population where a rise in multimorbidity and associated consequences will require proactive, upstream, person-centered care, multidisciplinary primary care approach to early identification of geriatric syndrome, assessment, and management 30.
